How do I maintain my monthly Health Care coverage?

Once you become eligible, you remain eligible as long as you continue to work or be directly paid for at least 280 hours each quarter by a participating Employer and the Trust Fund receives the required Contributions paid on your behalf. The 280 or more hours you work in any given quarter provide eligibility beginning the second month after the quarter in which the hours are worked. (reference above schedule)

When do I become eligible for benefits?

You will become Eligible on the 1st day of the 2nd month after completing a calendar quarter of employment, having worked 280 hours or more in that quarter.

Non-bargaining employees: Month-to-month eligibility – Eligible the first day of the second calendar month following required contributions (Flat Rate).

Who are my eligible dependents?

Dependent includes only an Employee’s:

a. legal spouse

b. children, including a legally adopted child or a child placed in your care in anticipation of adoption where the adoption is being actively pursued, foster child, a step-child who resides in the Employee’s household, and a child who is the subject of a Qualified Medical Child Support Order:

(1) who is less than age 26; or
(2) who is otherwise eligible, but reaches age 26 while covered by this Plan and is at that time incapacitated and for whom this Plan has received proof of incapacity:
(i) within 31 days after reaching the age limit; and
(ii) Annually thereafter.
