When will my eligibility terminate (Loss of Eligibility)?

Your coverage will automatically end on the earliest of the following dates:

1. the date this Plan ends;
2. the date you become ineligible for coverage;
3. the date you begin active duty in the armed forces;
4. the date you fail to make a required premium contribution; or
5. 31 days after you have established residence outside the U.S. or Canada.

Your eligibility for benefits terminates on the final day of the forth month following the quarter in which you worked less than 280 hours. or where your employer fails to pay a required contribution on your behalf. If, for example, you last worked 280 hours in the Oct-Nov-Dec quarter, your eligibility will terminate April 30^th, unless it is continued by COBRA self – payments or Extended Coverage.
